This write-up describes the system intent and engineering approach for an active build. It does not claim shipped functionality, measured performance, regulatory approval or customer outcomes that have not been published.
Design objectiveMove multimodal records through a traceable, governed data lifecycle
A traceable path from input to outcome.
- 01Receive
Accept a source package with an explicit transaction manifest.
- 02Validate
Check identity, schema, permissions, integrity and required metadata.
- 03Preserve
Keep source originals immutable and link every derived artifact to its origin.
- 04Transform
Run OCR, speech or other extraction as versioned, reviewable processing.
- 05Govern
Expose only authorized records with a complete audit and provenance trail.
Treat a data handoff as a transaction
Clinical and research evidence arrives in different forms and from different systems. A file transfer that finishes is not necessarily a valid data transaction: the package may be incomplete, duplicated, linked to the wrong encounter or stripped of the context needed to interpret it.
ClinTx Engine is being developed as a data transaction engine and central multimodal data centre. Its job is to make each intake, transformation and handoff accountable while preserving the distinction between a source record and a derived interpretation.
An auditable multimodal lifecycle
The proposed intake begins with a transaction manifest that records the source, sender, received time, expected items, data type, schema or format version, integrity checksum and applicable access policy. Processing is idempotent: repeating the same delivery should not silently create a second clinical record or overwrite the original.
After validation, the system can link structured records and referenced objects to the relevant patient or encounter using an approved matching process. Documents, scans, images, audio and extracted text remain separately identified so that a reviewer can distinguish the original evidence from OCR, speech recognition or another derived artifact.
- Reject or quarantine a package when required fields, integrity checks, consent state or identity resolution are incomplete.
- Record each transformation with its software/model version, parameters, timestamp and confidence or exception state.
- Keep the original object immutable; store normalized metadata and derived outputs as linked, versioned records.
- Return a transaction receipt with accepted, rejected or pending items and actionable reasons for every exception.
Multimodal processing without losing context
A central data centre should index clinical metadata and relationships while storing large source objects through a controlled object-storage layer. The exact clinical systems, file formats, hosting region and retention schedule remain integration decisions for the project partners; adapters should not assume a single vendor or a single record format.
OCR, speech recognition and language normalization can make material easier to search, including bilingual voice data where the project scope requires it. Those outputs must retain links to the source page, image region or audio segment and a confidence indicator. Low-confidence extraction should enter a human review path instead of being treated as verified clinical fact.
Privacy, access and clinical boundaries
The system should enforce least-privilege access, tenant and study boundaries, encrypted transfer and storage, auditable access events, and partner-defined consent and retention rules. Identifiers used for matching should be handled separately from analytic content where the deployment permits that separation.
ClinTx Engine is described here as data infrastructure, not a diagnostic or treatment recommendation engine. No regulatory approval, production deployment or compliance certification is claimed. Data residency, applicable legal requirements and the exact control framework must be confirmed with the organizations that will supply and use the data.
Validate the transaction, not just the extraction model
The evaluation plan should cover data preservation, linkage and transaction behavior alongside OCR or speech quality. A highly accurate extractor does not compensate for an incorrect patient match or a missing source record.
- Test package completeness, checksum integrity, duplicate delivery handling, idempotency and recovery after interrupted transfers.
- Measure identity-link precision and the rate of records routed for manual resolution; false matches should be treated as a high-severity failure.
- Evaluate OCR and speech recognition on partner-approved, representative samples, stratified by modality, language, noise and document quality.
- Audit whether every derived field can be traced to a source object and processing version, and whether access events match the approved policy.
Current build focus
The first engineering milestone is an agreed transaction contract and a controlled pilot with representative but approved data. That will settle the source formats, identity rules, required metadata, error states and review responsibilities before the system expands to additional modalities or workflows.
